Item missing data is not uncommon in self-report surveys. The standard SF-12 scoring algorithm, however, is a weighted sum of the SF-12 items and requires answers to all 12 individual items in order to calculate PCS and MCS summary scores. However, missing items are likely to occur in any study that collects data by self-administration. Studies have shown that among patients with missing SF-12 items, approximately 50 percent have missing data on only one of the 12 items (Ware et al. 2002). Yet, by using the standard SF-12 scoring algorithm, even one missing item will yield missing data on the SF-12 PCS and MCS. The loss of observations because of missing SF-12 items can reduce analytical power, increase variation in parameter estimates, and potentially lead to bias, particularly when the SF-12 is the primary outcome of interest in a study. Therefore, an algorithm that can efficiently and accurately estimate PCS and MCS scores with partially missing SF-12 data would be valuable to researchers. Since the SF-12 PCS and MCS scores are directly weighted functions of SF-12 items, the amount of missing data will have a significant impact on the imputed value. Hence, the imputation algorithm may need to apply different models according to the degree of missingness. As the number of missing SF12 items increases, the available information and predicting power from SF-12 items diminishes. When the number of missing SF-12 items reaches a certain level, the available SF-12 items may no longer carry sufficient information to produce accurate parameter estimates of SF-12 PCS and MCS scores. In these situations, additional patient information may help in estimation. We applied a multi-pattern regression-based algorithm for imputing SF12 PCS and MCS scores. This algorithm fits different imputation models according to the amount of SF-12 information missing. These imputation models are either a function of the available SF-12 items alone, or are a function of the available SF-12 items plus additional patient characteristics. Through simulation analyses using baseline and follow-up data 2 years later, we have evaluated and validated the imputation algorithm.

Turkey and Greece would undoubtedly have come to war over Cyprus had it not been for 1) the fact that the Greek military junta responsible for the coup surrendered power a few days after the Turkish invasion, and 2) the pressure put on Turkey and Greece by their fellow NATO members. If Cyprus is ever to be reunited, some way will have to be found that is not strictly along democratic lines. Democracy means majority rule, and the Greek Cypriots, as one could expect, are all for it, as are the Protestants of Northern Ireland and the blacks of South Africa. But the Turkish Cypriots, understandably, wont accept such a governing formula.

In her introductory essay, Anne Wilkes Tucker, Curator of Photography at the Museum of Fine Arts, Houston, provides a historical context for the catalogue through a survey of Western publications on and exhibitions of Japanese photography. As Tucker points out, the history of Japanese photography has had a relatively short existence as a field of study in its own right, even within Japan. It was not until well after World War II that the discipline garnered much serious interest and not until the 1980s that widespread institutional support allowed for significant advances in research. Since the 1970s, the work of Japanese photographers has been featured in a number of exhibitions and books in both Japan and the West. However, much of the focus has been on post-World War II photography. The most comprehensive English-language publication to date was A Century of Japanese Photography, 1840-1945, a translation of a 1971 Japan Photographers Professional Society volume, published in 1980 with an introductory essay by John W. Dower. It is worth noting that even within Japan an exhibition covering the scope of the Houston show has never been organized. Tucker articulates the need for a nation-based survey of Japan, an issue sure to be a main point of contention among critics of this text. Asserting that most Japanese photography recognized in the West has been assimilated into an international frame of reference that pays little attention to the national traditions and contemporary context from which it arose (2). Tucker argues that such interpretations limit understanding and needlessly [diminish] the levels on which the work may be approached (13). The catalogue presents a standard chronological survey in six essays by five noted scholars. Kinoshita Naoyuki, Professor of Cultural Studies at Tokyo University, examines the second half of the nineteenth century in The Early Years of Japanese Photography. Portraits and landscape comprised the earliest genres, along with the scenic views and contrived images of Japanese customs made for Western audiences, while later uses included documenting frontier expansion and collections of national treasures. Kinoshita highlights some uniquely Japanese uses of photography as well. These included the promotion of the study of photography by Japanese authorities during the 1850s and 1860s as one of many branches of Western technology to be mastered in order to modernize the country (22-23), and photographic portraits as replacements for miei, traditional painted portraits of the deceased (27).
The photography collection at the Museum of Fine Arts, Houston is a nationally recognized assemblage of over 20,000 photographs by approximately 3000 artists that encompasses dozens of major collections.

As Gemstar-TV Guide International Inc. re-launches a bigger version of its television programming guide, the company faces rising legal costs associated with a Securities and Exchange Commission inquiry of its former officers. Several former officers, including founder Henry Yuen, are under investigation by the SEC for inflating licensing revenues. The trial has been put on hold as both sides battle over whether their severance payments may be frozen. For the April-June period, Gemstar reduced legal expenses by $5.8 million to $600,000. But the company also filed a complaint last month against its former insurance carriers alleging they have failed to pay claims filed under its policies.
The report also noted that the countys payouts in medical malpractice lawsuits dropped from $15 million to $9 million and general liabilities fell from $41 million to $13 million.

Certainly not a remarkable lady called Anne Scheiber, whose exploits in the stock market have just come to light. For those of you who may have missed the story, this is the lady who spent her working life as a tax clerk in the Internal Revenue Service (the American equivalent of our Inland Revenue) and who took up investing only in her fifties. Armed only with her meagre income, and despite being completely untutored in the finer arts of investment, Miss Scheiber was so successful at investing her modest spinsters mite that when she died earlier this year at the age of 101, she had turned her initial savings of $5,000 into a fortune worth no less than $22m. Her dividend income alone came to $750,000 a year by the time of her death, completely dwarfing anything she had earned in her working life. It is a remarkable story from which one can draw several morals. One is the obvious one that so-called experts have no prerogative on wisdom when it comes to investing in the stock market. Common sense and application can take you a long way. In Miss Scheibers case, her success stemmed from following some simple rules - investing in a limited number of well established companies that have a strong business franchise and the ability to generate significant amounts of cash now. Her biggest holdings were in proven cashgenerators such as Coca-Cola and the drugs company Bristol- Meyers Squibb. A second, related, moral is that it pays to be patient. Putting money into smaller, more speculative companies will often generate bigger returns in the short run, but for those willing to take a long-term view, there is nothing to beat buying shares in consistent money-spinners that have the capacity to pay handsome dividends. The returns over time can be just as big, and the risks are much reduced, particularly if you have personal knowledge of the company and what it does.
The sheer growth and diversity of the market has created problems for investors, including those who already have a portfolio of PEPs and those standing on the brink of investing Personal equity plans have proved to be a popular investment choice, offering scope for capital and income growth free of income and capital gains tax. Over pounds 20bn has been invested in some 5 million PEPs since their introduction in January 1987, and with over 1,000 on the market, investors have a bewildering array to choose from.
That, in essence, is the conventional argument on where the private shareholder stands. According to Sir Mark Weinberg, chairman of a committee set up by the Stock Exchange to examine whether the private investor is a dying breed, there is some truth in all this. But his committees report, published last month, predictably got a poor press, with most commentators saying that he had failed to come up with enough specific recommendations to reverse the trend. Sir Marks real offence seems to have been that he failed to criticise strongly enough the Stock Exchanges new rule allowing companies to exclude private investors from most new issues if they so choose. It was this decision which originally prompted the setting up of the committee. In addition, he said it was largely up to the financial services industry, rather than the Government or the Stock Exchange, to come up with ways of tempting more private shareholders back into the fold. Neither conclusion was guaranteed to win any plaudits from a constituency which prefers decisive-looking actions to well-meant words, and which has long marked the Stock Exchange down, not entirely without justice, as an enfeebled and not very effective organisation. Yet, in my view, Sir Mark is largely right on both counts. In fact, I would go further in saying that the outlook for private shareholding is probably brighter now than it has been for some time - partly because of new technology, and partly because of the recent changes in the financial services industry, which have introduced much more effective competition.

The story of golf is as much a tale about change as it is tales about great players miraculous shots and dramatic victories. Great players have been the constant in a game that has changed dramatically over the last 700 years. From wooden balls (prior to 1440) to feather-stuffed leather-covered balls known as “featheries” (1440 to 1848) to gutta percha balls made from the sap of trees indigenous to Malaysia (1848 to 1901) to rubber balls made from winding rubber thread around a solid rubber core (1901 to the present) to the solid core balls commonly used today the game has relentlessly evolved. And of course as balls changed so did clubs. The first clubs had shafts made of wood probably ash. Wood clubs had long broad heads while the irons tended to have large faces square toes and nicked sockets. Sometime during the feather ball period perhaps in the late 1770s club-making became profitable enough that artisans began taking up the craft. By the 1800s most shafts were made from ash or hickory while all sorts of woods–beech pear apple–were used to fashion the heads of wooden clubs. Depending on the course the number of holes also varied usually ranging from five to 18. The shape and size of club heads also regularly were changed in the early years of the game either to improve accuracy or increase distance or both. As time passed the number of clubs also increased: from two or three in the 1600s to four or five in the 1700s to five or six in the 1800s to eight or nine or even ten by the late 1900s. Today a normal set of clubs numbers 14 and can be made of materials with space age sounding names like graphic tungsten and titanium. Every year hundreds of companies produce thousands of variations of golf clubs in what appears to be a never-ending technological war all with the purpose of helping a golfer hit a little white ball toward a very small hole an awful long distance away.

I, myself, a resolute aficionado of modern art, have occasionally shied away from the too unusual. One time in Seattle, at the University of Washingtons art gallery, I was invited by a gallery docent to remove my shoes and walk through a conceptual work of art that I found distasteful, if not disgusting. The drama of mergers, acquisitions, downsizing, and the managed redirection of health care resources away from acute care hospitals and high-technology medicine has overshadowed another change in the contemporary health care culture (Duva, 1990; Edmonds Hallman, 1995; Gabel, 1997; Salmon, 1995). The purpose of this article is to discuss the growing interest in alternative health care and offer an explanation for this phenomenon. Alternative health care is not easily defined. Often it is explained by describing what it is not — the scientific practice of nursing or medicine as it has come to be understood in first world countries during this century. Physicians who are sympathetic to the movement say that alternative therapy encompasses a broad spectrum of practices and beliefs (Eisenberg et al., 1993), and reflects a holistic world view of health and illness that is outside the biomedical mainstream of medicine (Rauckhorst, 1997). Other writers compare and contrast traditional and alternative medicine as a way of defining its practice. Western medicine is specialized; alternative care is holistic. Scientific practice is evaluated by its outcomes and its coherence with protocols; alternative therapy is not in conformity with the standards of the medical community and is considered to be unorthodox. Little is known about its safety, efficacy, or effectiveness (Gevitz, 1988; Lohr, 1997; U.S. Congress, 1990). The science of medicine is taught and practiced in universities and hospitals; alternative care is not widely taught is U.S. medical or nursing schools and is generally unavailable in American hospitals (Eisenberg et al., 1993; Engebretson Wardell, 1993). Insurance companies and federal and state governments pay for traditional medical care. With few exceptions (acupuncture and chiropractic medicine), alternative therapy has not been included as a health benefit by health insurance companies (Eisenberg, 1997). Recently, however, Oxford Health Plans announced its intention to offer coverage for alternative health care and provided its enrollees with a list of providers and services (Oxford Launches, 1998). There is an elaborate certification and licensing system for health care providers and hospitals; the world of alternative therapy is unregulated and its practitioners are usually unlicensed and uncertified (Eisenberg, 1997).
